Disability Services Access Coordinator – Short Breaks Team | 10153 | Secondment Opportunity / Temporary Fixed Term Contract until July 2027 | 37 hours per week | £37,280 to £39,152 per annum | Grade I | County Hall
The Short Breaks Team currently supports over 3,500 children with disabilities aged 5–17, and we are seeking a full time Disability Services Access Coordinator to cover a period of maternity leave until July 2027.
Families self refer to our service, and we receive over 100 new applications every month, alongside reviewing each child’s Short Break Plan annually. In addition, we provide advice and guidance to Social Workers, practitioners, and a wide range of professionals regarding Short Breaks, eligibility, and commissioned services.
As a Disability Services Access Coordinator, you will play a key role in ensuring that families receive a fair, transparent and needs led Short Breaks offer. You will review assessments, identify support needs, and work directly with families to ensure service providers are meeting expectations. You will also act as the liaison officer for a number of commissioned Short Breaks providers, supporting contract monitoring, performance reporting and service development.
You will be the first point of contact for families and professionals requiring more detailed information about Short Breaks, assessments, resource allocation, or issues with service provision. You will respond to telephone calls, emails and portal requests from families both applying for or currently receiving Short Breaks.
In this role, you will:
- Review child and family assessments to identify need and inform Short Breaks provision
- Ensure families can access services within the Short Breaks offer
- Work directly with families where support arrangements are not meeting need
- Resolve concerns or complaints linked to commissioned services
- Monitor allocated commissioned providers and report on contract performance
- Identify gaps in service delivery and contribute to service improvement
- Manage resources within agreed budget allocations
- Analyse data and support performance reporting across the service
You will need to enjoy working as part of a team, be able to prioritise a busy workload, and respond flexibly to the needs of the service.
Skills and knowledge you will need:
- Strong communication skills and confidence working with families and professionals
- Knowledge of the Children and Families Act 2014 and relevant disability legislation
- A good understanding of child development and safeguarding processes
- Experience supporting children, young people and families
- Experience using assessment to inform service provision
- Confidence using IT systems and maintaining accurate data
- The ability to work within budgets and support contract monitoring
There is a requirement to work at County Hall at least one day per week, with some travel across the county to support with Short Breaks information sharing and engagement sessions.
