As a member of the Disabled Children’s Referral and Access Team, you will be the first point of contact for families and professionals seeking support for disabled children who require a social care assessment. You will play a key role in enabling access to Short Breaks under the local offer, helping young people to achieve greater independence and improved outcomes.You will work collaboratively with the Children with Disabilities Teams, the Young Adults Team, and our in-house short breaks providers to ensure seamless support. The role also involves partnership working with health and education professionals as well as a range of private and voluntary sector organisations. About you We’re seeking a confident and compassionate individual who builds strong relationships and networks with ease. You work effectively with children, ideally bringing experience from the disabilities sector, and you take pride in staying organised and managing administrative tasks efficiently. You actively engage in multi-agency collaboration, prioritise safeguarding, and thrive as part of a supportive, team-focused environment.Experience in social care or disability services is desirable but not essential as we recognise the importance of transferable skills and we provide a comprehensive, focused induction.For a full list of the requirements of the role, please check out the Job Description & Person Specification.
