  • Deliver training, housing surgeries, guidance and practice tools to build practitioner confidence in homelessness prevention, housing options, tenancy sustainment, affordability, statutory duties and the Leaving Care Housing Protocol.
  • Work alongside practitioners on complex cases involving placement breakdown, eviction, rent arrears, sofa surfing or safeguarding concerns, coordinating multi-agency prevention plans and rapid responses when homelessness occurs.
  • Liaise with housing authorities, providers and support services, unblock accommodation issues, support protocol implementation, identify emerging trends and risks, and drive continuous improvements to outcomes for care leavers.

You will need

  • Knowledge of housing, homelessness prevention, safeguarding and relevant legislation, or the ability to develop this quickly.
  • Experience of supporting children, young people, care leavers or vulnerable adults to achieve positive outcomes.
  • Strong communication and partnership-working skills, with the confidence to advise, influence and support others.
  • The ability to build positive relationships with young people, colleagues and partner organisations.
  • Excellent organisational skills, with the ability to manage your own workload and respond to changing priorities.
  • A commitment to improving outcomes for care leavers and young people at risk of homelessness.
  • A relevant qualification or equivalent experience in housing, social care, youth support or a related field.
  • The ability to travel across Suffolk as required.
  • Desirable
  • Experience of working within Children’s Services, Leaving Care, Housing, Homelessness, Supported Accommodation or a related service.
  • Experience of delivering training, advice or practice support to professionals.

The team

The Housing Transitions Advisor sits within the Housing Development Team, working closely with Leaving Care, Staying Close, Housing Related Support (HRS), district and borough housing authorities, commissioning teams, and accommodation providers. The team is dedicated to improving housing outcomes for care leavers and young people at risk of homelessness through early intervention, strong partnership working, and system-wide service improvement. By combining specialist housing expertise with direct support and workforce development, the team helps ensure young people can access, maintain, and thrive in safe, suitable, and sustainable accommodation.

We offer a fantastic and inclusive working environment, with diverse and active staff networks, flexible working options, and a wide range of employee benefits. Most importantly, you’ll have the opportunity to make a meaningful difference to the lives of children, young people, and families across Suffolk.

We want to represent the diverse communities we serve. We welcome applications from all individuals, especially those from groups that are currently underrepresented in the organisation, as shown in our Workforce Equality Report.
