We are recruiting a Link Worker to join the High Impact Use (HIU) team. This service aims to improve the outcomes of people who are frequently using NHS urgent and emergency care.Working across Cambridgeshire and Peterborough, the Link Worker will work directly with people who have accessed hospital Emergency Departments frequently and whose needs are likely to be complex but non-medical. The aim is to better understand each person and their needs that bring them to ED, but which cannot be met there. Joining an established team, the Link Worker will support adults who are using a lot of unplanned services but no one service has been able meet their needs.Using a personalised care approach, this support will be offered via a mix of listening, practical help and advocacy and/or connection with wider system services. Creative and innovative approaches to support are encouraged. Direct work will take place in a variety of venues including people’s homes, community venues or digitally. About you Full training will be given but the following experience is required for the role:You will have a good understanding of health inequalities and the wider determinants of health.You will have significant experience as a case holding practitioner/worker, with an approach that aligns with personalised care. You will have experience of partnership working and navigating services.
